Improving Sickle Cell Disease outcomes through collaboration and innovation
A Learning Health Network for Pediatric Sickle Cell Disease
The H.E.A.L. Network is a Learning Health Network (LHN): a collaborative system that brings together diverse and interested parties to generate and apply knowledge in real time to improve patient outcomes and the overall healthcare system.
We envision a world where every child with Sickle Cell Disease receives equitable, timely, and evidence-based medical care, driving improved outcomes and reducing disparities in healthcare access and quality.
Connecting real-time for enhanced care
The H.E.A.L. Network is bringing together patients, families, clinicians, researchers, health system leaders, and community partners in the shared goal of improving Sickle Cell Disease care and outcomes. With an “All Teach, All Learn” mentality, and commitment to continuous learning and iterative innovations, we will bridge gaps in care and enhance clinical practices.
Built on quality improvement and research
The backbone of the H.E.A.L. Network collaboration allows participating organizations to access aggregated, de-identified patient data. This data can then be used to analyze performance and inform opportunities for improvement and research. In addition, the H.E.A.L. Network offers training in quality improvement (QI) methods, which will empower organizations to address their specific challenges.
Improving patient outcomes at scale
The American Academy of Pediatrics recognizes Learning Health Networks (LHNs) as one of the most effective strategies to achieve population health outcomes at scale. As with other successful LHNs, the H.E.A.L. Network will work to improve SCD care by integrating data-driven insights into practical clinical outcomes in real time across multiple healthcare institutions. Our network currently includes founding members from 6 different healthcare institutions across the Midwest and Northeast. If you are interested in learning more about the H.E.AL. Network as we continue to expand, contact us.
